In the United States, it would be decided by state law. However, it's courting disaster, litigation and it's counter intuitive to the mission of a hospital. Most hospitals don't allow smoking even outside by visitors or employees. read more
Legally speaking though I think there is nothing against having an open bar on hospital premises.. Doctor's lounges can have a restricted access bar, where docs can gather after work should they wish to for a drink, or nurse their soft drinks while on call. read more
