I use Colemak on OS X which likely has behavior similar to Dvorak. I can't speak for hardware-based Dvorak keyboards (rare) or Windows. Basically the experience is very good. read more

Even though you can keep the keyboard shortcuts the same, this is not the case when you need to use Ctrl + [letter] on a Mac. The OS will stick to Dvorak, so terminal commands and editing files in vi will need to be done in Dvorak. Some programs do not take the Dvorak -- Qwerty ⌘ setting. read more

After initally switching to Dvorak, I kept a cheat sheet beside my monitor so I could check which key I needed if I forgot. Since the A and M keys are the same in Dvorak as in QWERTY, those were the ones I could remember easiest.
